Holy Family Youth Ministry - Parental Permission Form

Thank you for bringing your child/ren along to Youth Group. We hope that their time in youth group will help them as they grow in faith, allow them to forge great friendships and have a lot of fun along the way. We have already asked the young people to give us some basic contact/personal information, but would appreciate your confirmation of important contact details and permissions as detailed below.
